Thank you very much to the parents that replied.  We are starting Patrick on
5mg Lisinopril on Sat.  Based on all your comments, I feel much better about
him being on this medication and the prognosis.

Carla, I'm sorry to hear that your son's kidneys have had such a rough go.
Poor kid.  Sound like he has a great mom looking after him.  You asked about
how the urine samples were collected.  Here's the answer:  The initial
sample was a random test that indicated a higher level (just using a test
strip).  That has been followed up with two 24 hour urine tests as well as
two morning urines and another random sample (at the nephrologist - which of
all the samples was in the normal range).  So unfortunately it's pretty
conclusive and we need to move on to the next step

You mention about having to measure his blood pressure.  Have you had issues
with dramatic changes in BP?  How about side effects?  My son is freaked out
about swelling and dizziness.  He just started driving and knows that
dizziness could put a kibosh on that.

My 11 yr old son has had microalbuminuria for over 4 yrs and is on an ACE
inhibitor. 
Unfortunately Cam had a rare form of kidney cancer when he was three and
developed high blood pressure because of that and then he was diagnosed with
diabetes a year later. Microalbuminuria sounds scary and had me worried
sick, but he hasn't had any trouble... I do worry because his poor kidneys
have 3 strikes against them. 
Was your son's urine test a random test or an overnight collection? I would
not rely on a random test. Overnight collections ("first morning
collection") are good if they stay in bed all night and go straight to the
bathroom first thing in the morning without doing any other activity or
walking around much. They even had Cam do a 24 hour collection, but that's a
hassle. I believe anything lower than 30 is norm for microalbumin. Cams was
as high as 67 and as low as 4. At his last check-up in July, his A1C was up
to 8.1 (the highest since diagnosed with diabetes thanks to puberty!) and
his microalbumin was up to 29. When we found out he had diabetes in 2002 his
nephrologist switched his BP meds to an ACE inhibitor (2.5mg tab enalapril)
to protect his kidneys and they bumped it up to 3.75mg when he started
having problems with microalbumin and that helped a lot. Cameron has grown a
lot and I think it could probably be bumped up a little more. He goes back
in Nov, so we'll find out more then.

Your son's BP is good and you may have to monitor his BP daily if he is put
on an ACE inhibitor, but I think a small dose would help. But, I'd be very
interested in knowing how many urine tests your son has had and if was first
morning urine??
A lot of exercise, being sick, high BG's etc can effect it.
